Questions

Questions about care plan

What is a care plan?

A care plan is a written plan agreed with your care team that sets out your health goals, treatments and next steps.

Who writes my care plan?

Usually you and your care team do it together. Your own goals and daily life should shape it.

Why should I review my care plan regularly?

Your health and needs change. Reviewing it helps make sure it still fits you.

What should I do if I cannot follow part of my plan?

Tell your care team. They can adjust it to something realistic rather than leaving you to struggle.

Why set a lifestyle goal?

A clear goal gives you something to work toward and makes progress easier to see. Small goals you can keep are better than big ones you drop.

How do I choose a good goal?

Make it specific and realistic, like a short walk after lunch on weekdays. Start small and build up.

What if I miss a day?

Missing a day is normal. Pick up again the next day rather than giving up.

Should I check with my care team before changing my activity or diet?

Yes, especially if you have a condition, take medicines or have not been active for a while. They can suggest a safe starting point.
